Health dept. releases food inspection report

Critical violations are defined as a provision of the Retail Food Establishment Sanitation Requirements, Title 410 IAC 7-24 that, if in noncompliance, is more likely than other violations to significantly contribute to food contamination, illness, or environmental health hazard. Example: Hot food not being held at 135°F. or warmer; cold food not at 41° F, or below; or food contact surfaces contaminated.

Non-Critical violations are provisions of the Retail Food Establishment Sanitation Requirements, Title 410 IAC 7-24 that, if in noncompliance, is not likely to significantly contribute to food contamination, illness, or environmental health hazards but are issues of concern. Examples: No hair restraints; or mop being left in a mop bucket rather than being allowed to air dry.

Repeat violations are issued when a follow-up inspection finds that a previous issue has not been corrected and the establishment continues to be noncompliant and presents a hazard.

Corner Food Mart, 51 West Walnut Street, NV: 2 Critical: No valid serve safe, no sanitizer for wiping tables. 10 Non-Critical: No hair restraints, wiping cloth not in sanitizer, no hand towels at the hand washing station, no soap at the hand washing station, equipment not maintained, mold inside of the ice maker, food improperly stored in the freezer, door seals on the pizza make table have a build up of debris and mold, dumpster lid open, mop improperly stored, chemical test strip not available.

Hoosier Street Grill, 161 North 5th Street, NV: 3 Critical: Hand sink being used for something other than hand washing, no serve safe employee, chemical sanitation not available. 2 Non Critical: No hair restraints, no soap at the hand sink.
Lumberjacks 5530 East Hwy 50, Butlerville: 2 Non Critical: Door seals on the make table need replaced, improper food storage. 1 Critical: Cutting surface rough or gouged.

Applications

Where are dates or time period for this report? Where is the report concerning what violations forced the Health Department to close the Dairy Queen? It's funny that one seems to be glossed over and we jump to a single non critical in this article. The Aug. 14 story by this paper regarding the forced closure suggested a followup would be forthcoming. Where is the report?
